The Resident Services Case Managers are available to assist individual residents living at Stoughton Housing Authority’s Elderly/Non-Elderly Disabled housing communities in connecting to resources and services from the community by:
Identifying benefit programs and assisting with enrollment
Providing assistance with lease compliance issues, re-certifications, and resident advocacy
Referrals to home care services and assistance with qualification procedures
Assisting with forms and paperwork
Providing assistance in identifying and accessing resources to support individual goals
